The Application Form
The Application Form for 2026 must be completed in full and signed on every page before your child can attend Little Star. Please note that not all PDF Readers will be able to use the form. Adobe Reader is about the best. If you have any problems, please use your PDF reader's built-in 'fill & sign' feature. Paper copies of this application form are available at the school on request.
Please make sure to use the CORRECT PAYMENT REFERENCE or your payment may go to another child.
